Salary Range:
Case Manager I: $40,809.60 Annually
Case Manager II: $43,305.60 Annually
Case Manager III: $46,009.60 Annually
Shift Available: Full-time
Essential duties and responsibilities:
- Staff will maintain ethical and professional communications at all times using respectful Trauma-Informed Care language and the model of care provided will be in alignment with the principals of a Certified Community Behavioral Health Center
- Provision of routine case management, assessments, care coordination, skills training, psychosocial rehabilitation, recovery planning, crisis intervention, engagement and documentation practices should be consistent with best practices, agency standards, and grant requirements
- Services are provided to adults using an integrated care approach to Wellness and utilizing the 8 Dimensions of Wellness
- Services will address behavioral health and co-occurring disorders that interfere with the individual’s functioning in the community and incorporate a holistic approach to recovery
- Work alongside with other treatment providers, first responders, and community providers in the rural communities to identify effective treatment methods, engagement strategies and community supports to sustain recovery
- Work as part of the treatment team including, but not limited to medical personnel, peer support, therapist, and other clinical support staff in efforts to provide services using evidenced based practices in the Crisis Respite and Recovery Center and in rural community settings
- Flexible scheduling to allow best access to people to be served
- Travel to the rural and frontier areas to provide outreach, education and treatment
- Adheres to TPC Business Code of Conduct and reports concerns if ethical and/or business standards defined in the code are compromised
Required qualifications to be successful in the job:
- Bachelor’s degree from an accredited university with a major in psychology, sociology, social work, counseling, medicine or other related human services field for QMHP credentialing
- For Case Manager I, no experience required
- For Case Manager II, one (1) year experience of behavioral health case management
- For Case Manager III, two (2) years experience of behavioral health case management
- Graduate work in a related field may be substituted for up to one (1) year of experience
- All members in the team will be trained in accordance with the evidence based practices requirements and TPC standards

- Must have a valid Texas driver’s license and be insurable under agency vehicle policy
- Must be 18 years of age or older
Preferred qualifications to be successful in the job:
- Degree in social work, psychology or license as a Chemical Dependency Counselor is preferred
