Medical Records

A flat fee of $6.50 will be charged when records are requested. This flat fee includes labor, supplies, and any applicable postage.
If TPC is requested to prepare a written explanation or summary in lieu of copies, you may be billed $75.00 per hour of preparation.
Note: If TPC receives an unusual or uncommon request, TPC may calculate the actual costs of responding to the request.
