Salary Range:
Case Manager II: $43,305.60 Annually
Case Manager III: $46,009.60 Annually
Shift Available: Full-time

Essential duties and responsibilities:
- Staff will maintain ethical and professional communications at all times using respectful Trauma-Informed Care language
- The Integrated Health Navigator works within a team which provides both primary medical and behavioral health care services and interventions in an outpatient primary care medical clinic.
- The position will be responsible for conducting appropriate assessments and intakes, and will also provide case management, primary-to-behavioral health and behavioral health-to-primary care navigation services; performs other psychosocial interventions, as needed.
- The Integrated Health Navigator will refer to community agencies as necessary and provide education and/or consultation to primary care staff to ensure achievement of quality outcomes.
Required qualifications to be successful in the job:
- Bachelor’s degree from an accredited university with a major in psychology, sociology, social work, counseling, medicine or other related human services field for QMHP credentialing
- For Case Manager II, one (1) year experience of behavioral health case management
- For Case Manager III, two (2) years experience of behavioral health case management
- Graduate work in a related field may be substituted for up to one (1) year of experience
- Must have a valid Texas driver’s license and be insurable under agency vehicle policy
- Must be 18 years of age or older
Preferred qualifications to be successful in the job:
- Two (2) years clinical experience in an outpatient behavioral health setting
- Bilingual
- At least one (1) year of experience working in a primary care setting
- General knowledge of community resources and the principles of case management
