Adult Behavioral Case Manager/Care Coordinator I-III [0320] E

Location & Program:
Clarendon, Texas – Adult Behavioral Health

Salary Range:

Case Manager I: $40,809.60 Annually
Case Manager II: $43,305.60 Annually
Case Manager III: $46,009.60 Annually

Shift Available: Full-time

Essential duties and responsibilities:

  • Staff will maintain ethical and professional communications at all times using respectful Trauma-Informed Care language
  • Serves as Care Coordinator for adults in the Amarillo/Clarendon service area experiencing behavioral health issues
  • Will be responsible for organizing consumer care activities and sharing information among all participants concerned with the client’s care to achieve safer and more effective care
  • Will connect client to identified community resources, provide follow-up with referrals and document activities according to agency policy
  • Will conduct state approved assessment, determine treatment recovery plans with consumers
  • Position will organize and provide culturally competent care activities and collaborating among the participants involved with the individual’s care
  • Additional duties can include: assessment & treatment/recovery planning, working within the team to ensure appropriate services are delivered, engaging family by taking a person centered approach to help identify strategies and objectives
  • This position will spend at least half of their working time directly working with the individuals assigned to caseload
  • Position will assist & collaborate with the interdisciplinary Behavioral Health Team in promoting recovery, skill development and community stability
  • Adheres to TPC Business Code of Conduct and reports concerns of ethical and/or business standards defined in the code if compromised

Required qualifications to be successful in the job:

  • Bachelor’s degree from an accredited university with a major in psychology, sociology, social work, counseling, medicine or other related human services field for QMHP credentialing
  • Degree in social work or psychology preferred
  • For Case Manager I, no experience required
  • For Case Manager II, one (1) year experience of behavioral health case management
  • For Case Manager III, two (2) years experience of behavioral health case management
  • Graduate work in a related field may be substituted for up to one (1) year of experience
  • Must have a valid Texas driver’s license and be insurable under agency vehicle policy
  • Must be 18 years of age or older
