Texas Panhandle Centers Behavioral and Developmental Health (TPC), formerly known as Texas Panhandle Mental Health Mental Retardation, is part of the Texas Community Center system and is governed by a local board of trustees representing the citizens of the upper twenty-one counties of the Texas Panhandle. We serve individuals with behavioral health needs (mental illness), intellectual and developmental disabilities and children with developmental delays.
At the heart of TPC’s work is our mission:
To respond to the diverse needs of all people who require behavioral and developmental health services by creating an accessible system of care which supports choices and results in lives of dignity and independence.
To individuals who attended the Equine Therapy event at American Legion Post 671, in Borger, Texas on January 28, 2017.
Individuals who participated in Equine Therapy were asked to complete a sign-in sheet and referral form asked for the following: Participant’s name; Last four of SSN#; Date of Birth; Address; Phone number; Gender; Race; Ethnicity; Military Status; and the type of therapy in which the individual would participate. The sign-in sheets and referral forms were subsequently misplaced. Texas Panhandle Centers conducted an investigation into the whereabouts of the forms but they were not found. Since discovering the loss of the sign-in sheets and referral forms, Texas Panhandle Centers has amended and improved upon the chain of custody of information at public events to decrease the risk of repeat occurrences. If you participated in Equine Therapy at the event referenced above and believe that your personal information may have been affected, you may contact Texas Panhandle Centers’ Privacy Officer at 806-351-3284.
